In Case of Emergency: For Rapid Response (Code Blue), pick up any facility phone and dial 7111 and announce Rapid Response to location of the code, i.e. room number, gym etc. For all other emergencies, dial 911.
Parking:
- Skilled Rehab: park in the parking lot behind building or on the grass outside the Care Center.
- TCU Parking: park in the parking lot behind the facility.
To Enter GCV Care Center and TCU: doors are open from 8 am- 8 pm. Before or after those times, press blue button and the concierge will open the door.
REHAB DEPARTMENT
Keys:
- Skilled Rehab: Located on top of the first picture to the right of the doors.
- TCU: Located in the fire extinguisher located on the wall to the right of the gym.
Clinical/Admin Forms:
- Skilled Rehab: Located on the wall in between the sides of the gym in the slotted holders.
- TCU: Located in the filing cabinet located in the back of the gym.
Schedules: Are in NetHealth. At start of your day, please reference the printed instructions in the office on how to print your schedule.
Standing House Orders: For all admits, a batch order is entered for PT, OT and SLP to eval and treat if needed.
Laptops: Float staff are to utilize the designated laptops on the rack located in the therapy gyms. The password to each computer is on the laptops themselves. Each employee is assigned their own laptop and is kept on their desks (please do not use unless you have no other option).
Blood Pressure Cuffs & Oximeter: In the therapy gyms there is a vital tower available for oxygen, BP, and HR monitoring. Oximeters freely in both gym as well.
Modalities: Modality forms are located in the file cabinet in the office. Ultrasound/estim machine is in the gym. Please refer to the Modality policy procedure in the Clinical Policy & Procedure Manual tab #38.
Speech Therapy Diet: Gulf Coast Village uses the IDDSI diets. Regular, Soft and Bite Size (SB-6), Easy to Chew (EC-7) and Puree. Thin liquids, Mildly Thick 2, Moderately Thick 3.
Diet Change Protocols: If there is a diet change, an Order is required to be completed in PCC and a diet slip should be filled out and given to the kitchen and a copy placed in hard chart (with RD assistance if floating in) and handed directly Then, a written communication form or email to the RD is expected to relay the change.
OT Cognitive Assessment Materials: CPT materials are available in the kitchen in the therapy gym (OTR/L only). Paper CPT and SLUMS forms are in the hanging files in Care Center or in the filing cabinet in TCU.
ADL Equipment: AE (reachers, sock aides, shoe horns, sponges, etc.) are located in the storage shed located in the Care Center gym on the far right side next to the cabinets.
Copy Machine Code: No code needed. Copy machine is located in the therapy office. *this is not a fax machine*
Therapy Times: Times (if needed) will be indicated on your schedule. Work with nursing to ensure that patients are getting their pain medications prior to therapy if needed. Suggested to check in with nurses and patients on schedule prior to beginning treatment in AM.
Nursing Mothers: You may use the Director of therapy office to have privacy.
Meal Times:
7:45-8:45am – Breakfast
12:15-1:15- Lunch
5:00- Dinner
At the end of the day therapists must:
- Complete billing and documentation in NetHealth.
- Complete progress notes if they are due for your scheduled patient.
- Kronos – be sure to have punch in/out, travel time and mileage (if appropriate)
Therapist completing evaluations:
- If you are completing evaluations, complete appropriate paperwork in NetHealth and all assessment tests.
- All TCU patients are scheduled 5x/week for OT and PT for part A. ST frequency as appropriate, just need to communicate with RD. Primary medical diagnosis code should be A00.0 for part A patients.
- Complete the “Therapy to Nursing Communication Form” for your discipline
- Place completed form on Karen’s desk or in the file holder in TCU.
- To complete chart review, go use Point Click Care in the Documents/Tasks section.
- OTRs to complete Barthel at eval (NetHealth and paper copy).
- At least one discipline needs to contact and have pt. sign the Welcome to Centrex form for any patient that is covered by part B. This form must be completed and signed by the patient/resident or POA if they are the responsible party. Place completed form on Karen’s desk. In addition, choose your own medical dx code for part B payors.
