In Case of Emergency or other need to page overhead:
In pt room: Step outside of the room, and call Nurse Stat to room location using a loud voice.
In Rehab Dept: Utilize Walkie Talkie to announce Nurse Stat to Rehab Dept
Parking: Park in the south parking lot and enter through the building’s main entrance door.
COVID-19 Procedures:
- During an Outbreak: Staff will enter through the main entrance, where they will be screened and temperatures taken before being allowed to work. Staff are required to wear surgical masks in common areas & N-95 masks w/ full PPE while in quarantined rooms. Rapid test required if not vaccinated or boostered.
REHAB DEPARTMENT
Key: There is a lock box on the rehab back door – combination is 2626. There is a key to unlock the back door in lock box. Make sure to re-lock door and replace key at end of day.
Therapy Gym & Office: Located to the right immediately after you come in through the front doors of the building.
Precautions Communication Sheet: will be by the phone in the therapy gym.
Forms: In 3-drawer metal file cabinet in therapy gym below. Soft charts for individual patients are in the therapy office filing cabinet in the Rehab Director’s office.
Schedules: The schedule sheets will be located on the white board in the therapy gym.
Laptops: Located on desks. Please plug in at the end of the day.
Blood Pressure Cuffs and Oximeter: In rehab gym near sink – in drawer labeled “Goniometers and BP cuff.”
Modalities: Ice packs in freezer in therapy gym, as well as freezer in dining room on rehab hall. US/Estim unit is in therapy treatment rooms. Hot packs and paraffin in therapy gym.
Nursing mothers: Speech therapy office – if SLP is on site at the time the room is needed, please let her know so she can gather what she needs before using.
Wi-Fi: SSID: VOANS1 Password: Ballington1896
If Wi-Fi out: ***Jet pack (with instructions) is in metal file cabinet by the therapy gym phone.
CPM Machines: None currently on-site.
ADL Equipment: Located in speech office in labeled cabinets.
Copy Machine: In therapy office or behind front desk at main entrance to building.
Secured unit entry: key pad located behind small picture on wall – code is 2023*
Meal times: Serving times varies with each resident depending on what time they prefer to eat within that time range. Serving times will vary by floor and wing. Here is the most recent posting
Breakfast: 7:00am-9:00am
Lunch: 11:30am-1:00pm
Dinner: 5:00pm-6:30pm
At the end of the day therapists must:
- During the week, contact the RD/designee if required minutes are not achieved for a person you were supposed to see. Complete documentation, billing, labor log and E-signatures in NetHealth.
- Complete progress notes, re-certifications or discharges and required g-codes if they are due.
- Clean up after yourself
- Plug in laptop in therapy office.
- Lock therapy doors and back door that enters gym if you’re the last person here.
Therapist completing evaluations:
- A Welcome to Rehab form must be completed and signed by the patient or POA if they are the responsible party. One form with each discipline checked. Remember to sign the back for Notice of Privacy Practices as well. The Welcome letter will be in the designated 3 drawer metal filing cabinet in therapy gym.
- Complete Evaluation in NetHealth, Section GG for managed care, and clarification orders.
Discharges:
Please do not discharge a resident unless the primary therapist specifies it.
