In Case of Emergency: Dial 400 on any facility phone, and call for a nurse to report to the site of the emergency.
Parking: Parking is available on the street in front of the facility or the lot behind the building. Do not park in any spaces that are marked as visitor parking. No staff should park in the garage.
Main Entrance: You can enter the building by the front or back doors. Both have key pad entries. The key pad is a black touch pad that lights up as you touch it. Code for on call staff 17240. At the front this key pad is located to the left of the door. There are surgical masks by these front doors as well.
COVID Protocols:
- Staff are no longer required to complete symptom screening upon entering the facility. If the facility is in an outbreak status, this will be indicated on a whiteboard to the left of the reception desk. While in outbreak, all staff must wear a mask, which can be obtained at the front door.
- Residents who test positive for covid may only be treated by staff wearing a fit-tested N95 mask. Staff who have not been fit-tested in the past year are unable to provide care.
REHAB DEPARTMENT
Take the elevators to the Basement level. Take a left around the corner off the elevator, and you will see the Wellness Center. The Therapy office is in the back left corner of the Wellness Center.
Key: If the Wellness Center is locked, there is a lock box to the right of the door. The code for the lock box is 2000. The keys open ALL of the doors in the wellness center. Additional keys are located on a hook in the uppermost cabinet near the office, in the OT kitchen area. It is a gold key and is labeled. Please lock the therapy office door if you are the last to leave. Please remember to return the key before you leave.
OT and PT Therapy Charts and Paperwork: All paperwork is in the therapy office file cabinets in the basement. There are patient soft charts in the top filing cabinet drawer to the left of the printer. PT/OT information is combined in a single chart for each patient.
Forms: Blank forms are located in the therapy office, in the bottom file cabinet drawer underneath the phone.
Schedules: All schedules are set in NetHealth. Printed copies of schedules can be found on the desktop by the printer in the therapy office. If a printed schedule is not available (weekends), this can be viewed via the “Care Provider Daily Schedule” report under the Reports tab in NetHealth. Need to be on Cassia-Therapy to print.
Standing House Orders: We have standing orders for PT/OT/ST eval and treat for Dr. Stein, patients on TCU.
PCC Access: We no longer have EMR access for float/oncall staff. Hard charts containing necessary information are found at the nursing stations on each unit. On 3rd floor, the nursing stations can be accessed using door code 12345. On 2nd floor, all other locations can be accessed using code 0073. Patient charts are in cupboards above the countertops in the nurse charting/dining areas.
Laptops: Laptops are stored on the back desktop in the therapy office. Please charge device when you leave.
Additional Treatment Spaces: There is a small treatment space to the left of the dining room on TCU. Parallel bars, nustep, therapy stairs, weights, walkers, etc can all be found in this room. There is also a box with folders for commonly used forms (FOMS, exercise handouts).
Blood Pressure Cuffs & Oximeters: BP machines can be found in the halls on the units. No designated BP machine for therapy. Oximeters can be found in the 3rd floor TCU treatment space, hanging from a hook in the upright storage cabinet.
Lymphedema Supplies: Located in therapy office, in the cabinet above the back right desk. Soiled wraps can be bagged and taken to the laundry room in the basement, or left outside the Rehab Director’s office to take to laundry.
Wheelchair Room: Located down the left hallway outside the Wellness Center. It is at the end of the hall on the right. Wheelchairs are labeled with the size on the lower left side by the back wheel. Leg rests are organized by type and R/L.
Nursing Mothers: There is a key for the Mother’s room in the first drawer across from the Rehab Director’s office. It is labeled ‘Mother’s Room.’ The room is down the left hallway outside the Wellness Center. It is near the end of the hall on the right side and is labeled. There is a refrigerator and sink for use in this room.
Stairwell: 2nd – 0073*, 3rd – *073, Wonder guard alarm – 0073
SLP Treatment Materials: SLP treatment materials are located in the basement office at the speech desk. Very few snacks are kept on the unit – these can be requested at the first floor kitchen (kitchen is by the elevators in the north admin hallway).
Modalities: Located on a cart in the wellness center. Please refer to the Modality Policy & Procedure in Policy Stat. On-call staff should not be initiating modalities on a weekend. Please return equipment back to where you found it.
OT Cognitive Testing Materials: Materials are in the bottom kitchen cabinet next to the fridge.
Copy Machine Code: None. The printer in the therapy office has a copy function.

Facility Layout by Floor/Unit:
- 1st floor is administrative office space, café, gift shop, and AL dining room. The AL memory care is on 1st floor as well. If you need to see someone on this unit, please check with Rehab Director/receptionist for door code, as it changes periodically.
- 2nd floor is Memory care and LTC
- 3rd floor is LTC and TCU; LTC unit is to the LEFT off the elevators, TCU is to the RIGHT
At the end of the day therapists must:
- During the week contact the RD/designee, or on the weekend contact the weekend manager if you are unable to see a pt for their scheduled visit. During the week, there is often an ‘unassigned’ pt list in the basement office for people that can be seen in the event of refusals.
- Complete billing and documentation for all visits in NetHealth
- Please make sure you have documented the appropriate treatment number in the 6th/10th visit tracker binders (located on the desktop by the printer)
