Balance Solutions identifies older adults at risk for falls and provides fall risk assessments, tailored treatments and fall prevention education classes.

Falls are a major health concern and affect one in five adults over the age of 65. There is strong evidence that falls can be prevented by screening to detect risk factors and prescription of tailored interventions. As a result, Centrex Rehab has developed a comprehensive fall prevention program called Balance First.

Centrex Rehab therapists screen all clients over the age of 65 for fall risk, and the program follows recommendations from the CDC fall prevention program, STEADI*, along with clinical practice guidelines from APTA.

Centrex Rehab therapists perform a therapy evaluation and then develop an individualized care plan to address specific needs. Using a problem solving approach, therapists work with the client, family and caregivers to address risks, develop priorities and change behavior.

Fall risk factors may include the following:

  • Strength deficits
  • Balance deficits
  • Mobility deficits
  • Medication errors and/or mismanagement
  • Vision impairment
  • Cognitive impairment
  • Depression
  • Effects of multiple medical conditions

Intervention to address specific fall risk factors may include:

  • A physical activity program for strength and balance
    A Matter of Balance program: eight-week workshop designed to reduce the fear of falling and increase activity levels among older adults
  • Gait training, including prescription and training using an assistive device
  • Training to make activities of daily living easier
  • Home environment assessment and modification recommendations
  • Cognitive assessment and training education on safety and fall prevention strategies
  • Referrals to other health care providers may include primary care physician, pharmacy, or vision or hearing professionals

In addition, we can provide education and/or consultation to our partner organizations and other care providers.
